Skilled Nursing
Skilled nursing services are provided by licensed Registered Nurses (RNs) in accordance with physician orders and the established plan of care.
Skilled nursing services may include, but are not limited to:

Comprehensive nursing assessments

Medication management and education
-
Wound care and dressing changes
-
Disease and symptom management
-
Monitoring of chronic and acute conditions

Patient and caregiver education
All skilled nursing services are delivered within the nurse’s scope of practice and in compliance with applicable regulatory standards.

Specialized Programs
We are prepared to support individuals through Indiana Medicaid Waiver programs, including Family Supports Waiver (FSW) and Community Integration and Habilitation (CIH), as permitted by program guidelines and authorization.

Personal Care
Home Health Aide (HHA) services are provided under the supervision of a licensed nurse and as outlined in the physician-ordered plan of care.
Home Health Aide services may include assistance with:

Personal hygiene and grooming

Mobility and transfer assistance

Activities of daily living (ADLs)

Basic safety support within the home
Home Health Aide services do not replace skilled care and are provided only as authorized in the plan of care.
Therapy services are provided only when ordered by a physician and included in the patient’s plan of care.
Therapy services may include:

Physical Therapy (PT)

Occupational Therapy (OT)
-
Speech-Language Pathology (SLP)
Therapy services are delivered by licensed professionals in accordance with state licensure requirements and clinical standards.

Our team may provide supportive services designed to help individuals maintain a safe, comfortable, and independent home environment, as authorized and in accordance with applicable licensure and certification requirements.
Services may include assistance with meal preparation, light housekeeping, errands, and companionship, depending on individual needs and approved care plans. We focus on delivering respectful, reliable support that promotes comfort, dignity, and peace of mind within the home.
Companion & Homemaker Services
Our clinical care services may provide professional medical support tailored to each individual’s needs, as ordered by a physician and permitted under current licensure and certification requirements.
Services may include IV therapy, laboratory coordination, and support for individuals managing chronic conditions, depending on eligibility, authorization, and approved care plans. Our licensed clinicians focus on delivering care in alignment with regulatory standards, prioritizing patient safety, dignity, and overall well-being.
Clinical Care
Hade Home Healthcare provides home health services in accordance with applicable federal and state regulations. All services are delivered only when ordered by a licensed physician and outlined in an individualized plan of care
Services are coordinated and overseen by licensed clinicians to support patient safety, quality outcomes, and regulatory compliance.
